Non-Employee/Affiliate Access Request Process

Kaweah Health requires the completion of a System Access Agreement for each entity that wishes to request remote system access. The execution of a System Access Agreement is required prior to individual system access request applications being submitted. Please note that each entity must only complete the System Access Agreement one time. Please email completed System Access Agreement to KDHubReach@kaweahhealth.org for review. You will be notified by the Kaweah Health Compliance Department if your System Access Agreement has been approved, at which time requests for individual user access may be submitted, see step 2. Requests for individual user access may be submitted by completing a Non-Employee User Access Application Here. Please reference the Non-Employee System Access Request Packet here for an overview of the request process.
Please Note
- System access applications will not be completed without the approval of a System Access Agreement.
- System request applications must be completed in entirety; incomplete applications will not be considered.
- All user requests will be reviewed independently to determine the need for the access.
- Kaweah Health reserves the right to approve, deny, or revoke access at any time.
- KD Hub training materials will be provided upon approval of application.
