Sub-Acute
Our patients have tracheostomies and G-tubes. About 1/3 of our patients are ventilator dependent. Most of our patients require full care, including skin protection and fall prevention. The unit is home to many of our patients. However, we have about 3-4 patients a year who recover enough to move on to a lower level of care or to Rehab.
Home Health
Hospital based Home Health agency employs nursing, physical, occupational and speech therapy, medical social worker and home health aide. Comprehensive medication review and instruction, wound assessment, treatment and teaching, infusion therapy, chronic disease management, palliative care. Home Health allows the independence and autonomy that is desired, while still fostering a strong sense of teamwork. Serve patients of all ages, including pediatrics. Extremely high patient satisfaction ratings and consistently below the National average for re-hospitalization rates. You will provide one-on-one care to patients with the unique experience of providing care for the patient in their own home environment.
Hospice
Hospice is a rewarding opportunity to care for adult End of life patients and Pediatric concurrent care. The home visits are meaningful to the families and can address needs immediately at a most difficult time of life. Staff can work 8, 10 or 12 hour shifts and every other weekend for some positions. Call is required for RN and LVN’s.
Private Home Care
Private Home Care, offers Personal Care Aides and Homemakers that fit your needs in the home. Services start with a visit from the manager to evaluate the homecare needs, assist you in designing your individual homecare program and inform you of community resources you may benefit from. Service include light housekeeping, laundry, meal preparation, supervisor, bathing, hygiene, and ambulation assistance. Services are flexible so you can decide how often you need services.
