Pre-employment Physical Exam/drug Screening
Employee Health Clearance
Before you begin your new role, you must complete a pre-employment physical exam with our Employee Health Department.
When your background is cleared, HR will be in touch with you to schedule the date, time and location of this appointment.
This appointment will include a drug screen, TB blood test & immunization screening, and physical exam to confirm that you meet the physical requirements of the position. The exam typically lasts one hour and may include a blood draw or immunization update. If a chest x-ray is necessary due to a prior positive TB test, please allow an additional hour to complete that test at the hospital following the appointment.
Please note that this appointment is restricted to you as the prospective employee; we are unable to accommodate guests. Children under the age of 18 cannot be left unattended in the lobby, so we kindly ask that you make necessary arrangements for child care.
What to Bring:
- Print & complete the Post-Offer/Pre-Placement Physical Evaluation Form prior to your physical exam and bring to your appointment. (Attached below)
- Bring photo identification at the time of your physical exam. Job offer is contingent upon passing a physical exam and drug screening.
- Bring your immunization/titers records, as we need to know your status of Measles, Rubella (MMR), Varicella (Chickenpox), Hepatitis B, Tdap, COVID-19, Flu (seasonal) as well as any TB testing done in the past year. For individuals with a previous positive TB test result, please bring proof of positive and a copy of the chest X-ray report if completed in the last 12 months. If you do not have your immunization records please visit https://myvaccinerecord.cdph.ca.gov or call your school or primary care physician.
- Please hydrate in preparation for urine drug screen and potential lab draws.
