Management
James McNulty, PharmD, Director of Pharmacy Health System
James McNulty, PharmD is the Director of Pharmacy Health System for Kaweah
Health. He oversees and provides strategic direction for both inpatient
and outpatient pharmacy operations and services. He has served as Director
of Pharmacy Services and Pharmacy Health System for more than 20 years.
He is Associate Dean for University of California, San Francisco School
of Pharmacy. He serves as a preceptor for PGY1 pharmacy residents in the
area of practice management. Under his leadership, the pharmacy team have
been nationally recognized multiple times for achievements in pharmacy
best practice, including ASHP Best Practice Awards in Anemia Management
in 2010 and Pain Management in 2014. Dr. McNulty received his doctor of
pharmacy degree from Idaho State University in Pocatello, Idaho in 1997.

Rheta Silvas, PharmD, Inpatient Assistant Director of Pharmacy
Rheta Silvas, PharmD is the Inpatient Assistant Director of Pharmacy for
Kaweah Health. She has been a clinical pharmacist for more than 25 years,
working at Kaweah Health for more than 20 years, serving first as the
inpatient pharmacy manager for over 10 years before moving into her current
role as assistant director. She currently serves as a preceptor for PGY1
pharmacy residents in the area of practice management. Dr. Silvas completed
her Doctor of Pharmacy degree at the University of the Pacific and a PGY1
pharmacy practice residency at the University of California San Diego.

Nicole Gann, PharmD, BCPS, Inpatient Pharmacy Clinical Manager
Nicole Gann, PharmD is the Inpatient Pharmacy Clinical Manager for Kaweah
Health. She is also a Clinical Assistant Professor (WOS) for the University
of California, San Francisco School of Pharmacy. She has worked at Kaweah
Health for over 20 years and currently serves as a preceptor for PGY1
pharmacy residents in the area of practice management and as the site
coordinator for pharmacy student rotations including students from University
of California San Francisco, School of Pharmacy. Dr. Gann completed her

Steven Richardson, PharmD, BCIDP, Clinical Pharmacy Coordinator
Steven is the Clinical Pharmacy Coordinator and PGY1 Residency Coordinator
for Kaweah Health. He is a current preceptor for the PGY-1 residency program.
Steven has worked at Kaweah Health since July 2018 and currently serves
on the RAPID Preceptor Development and Recruitment Committees. Steven
completed his BS in Biochemistry at Presbyterian College (SC), and his
Doctor of Pharmacy degree at Texas Tech University Health Sciences Center.
After schooling Steven completed a PGY-1 Pharmacy Practice residency and
a PGY-2 Infectious Diseases residency at the Cleveland Clinic in Cleveland,
OH. His educational interests include cardiac hemodynamics, fungal infections,
HIV, antimicrobial resistance, and pharmacokinetics. Email:

Hal Bradshaw, PharmD, Operations Pharmacy Coordinator
Hal Bradshaw, PharmD is the Operations Pharmacy Coordinator at Kaweah Health.
He serves as a preceptor for PGY1 pharmacy residents and pharmacy students
from the University of California, San Francisco. Dr. Bradshaw received
his Bachelors of Science in Human Biology from Brigham Young University,

Medication Safety
Mara Miller, PharmD BCPS, Medication Safety Coordinator
Mara Miller, PharmD BCPS is the Medication Safety Coordinator at Kaweah
Health. Mara received her Bachelors of Science in Biology and Bachelors
of Arts in Chemistry from California State University, Fresno in 2010
and then her Doctorate of Pharmacy from the Touro University in 2014.
She completed her PGY1 Pharmacy Practice Residency at Kaweah Delta Medical
Center in 2015. Prior to her current role, she practiced as a Clinical
Pharmacist in pediatrics/NICU, emergency medicine, critical care and internal
medicine. She currently serves as a preceptor for PGY1 pharmacy residents
and pharmacy students from the University of California, San Francisco.
Formulary Management
David Crowther, PharmD, BCPS, Formulary Management Specialist
David Crowther, PharmD, BCPS is the Formulary Management Specialist for
Kaweah Health and serves as a preceptor for both PGY1 pharmacy residents
as well as pharmacy students. He received his Bachelors of Science in
Physiology/Developmental Biology (2006) from Brigham Young University
and his Doctor of Pharmacy (2009) from Roseman University. Dr. Crowther
completed both his PGY1 Pharmacy Residency (2010) and PGY2 Drug Information
Residency (2011) at the University of Virginia Health System in Charlottesville
VA. After residency he worked as a Clinical Specialist in Drug Information
at MD Anderson Cancer Center in Houston TX for 6.5 years before joining

Internal Medicine
Bayode Fashemi, PharmD, BCPS, BCCCP, CGP
Bayode Fashemi, PharmD, BCPS is an Emergency Medicine Clinical Pharmacist
at Kaweah Health. He serves as a preceptor for PGY1 pharmacy residents
and pharmacy students from the University of California, San Francisco.
Dr. Fashemi completed his PGY-1 residency training at Baystate Medical
Center in Springfield, Massachusetts in 2012. Dr. Fashemi received his

Vata Hernandez, PharmD, BCPS
Dr. Hernandez is a Clinical Pharmacist primarily working in Internal Medicine
and Pain Management at Kaweah Health. She serves as a preceptor for APPE

obtaining doctorate of pharmacy from University of Tennessee in 2015,

Patrick Binaday, PharmD, BCPS
Patrick Binaday, PharmD, BCPS is a clinical pharmacist at Kaweah Health
primarily practicing in Internal Medicine and Pediatrics/NICU. He is also
an Assistant Clinical Professor at the University of California, San Francisco
School of Pharmacy. He serves as a preceptor for pharmacy students as
well as PGY1 pharmacy residents. After obtaining his Bachelor of Science
degree in Biology: Concentration in Microbiology and a Minor in Chemistry
from San Francisco State University, Dr. Binaday attended Midwestern University

Nibal Fadhil, PharmD
Nibal Fadhil, PharmD is an Internal Medicine Clinical Pharmacist at Kaweah
Health. After obtaining a BS in Biology-Biochemistry Concentration, and
a Minor in Chemistry, from the University of New Haven in CT in 2011,
she attended the University of Connecticut School of Pharmacy, where she
received her Doctor of Pharmacy degree in 2016. She completed her PGY
1 pharmacy residency at Waterbury Hospital in CT in 2017. Dr. Fadhil is
an Assistant Clinical Professor at the University of California, San Francisco

Brenda Do, PharmD, BCPS
Brenda Do, PharmD, BCPS is a Clinical Pharmacist at Kaweah Health. She

Jeanny An, PharmD, is a Clinical Pharmacist and serves as a preceptor at

Mimi Jakes, PharmD
Mimi Jakes, PharmD, is a Clinical Pharmacist at Kaweah Health. She serves

Dr. Jakes is from Erie, PA and received her doctor of pharmacy from Ohio

Lawrence Chang, PharmD, BCPS
Lawrence Chang, PharmD, BCPS is a Clinical Pharmacist at Kaweah Health.
He serves primarily as a preceptor for APPE students on Internal Medicine
rotations. After obtaining his Bachelor in Science in Pharmaceutical Chemistry
at the University of California San Diego in 2012, Lawrence received his
Doctor of Pharmacy at the University at Buffalo School of Pharmacy and

Lauren Clark, PharmD
Lauren Clark, PharmD, is a Clinical Pharmacist at Kaweah Health. She serves

Brooke Sabella, Pharm D, BCPS
Brooke Sabella, PharmD is a Pain Management Clinical Pharmacist at Kaweah
Health. She serves as a preceptor for PGY1 pharmacy residents and pharmacy
students from various schools. Dr. Sabella joined the pharmacy team as
a pain management specialist after completing her PGY2 Pain Management
and Palliative Care pharmacy residency at Summa Health System in Akron,
Ohio. She completed her PGY1 acute care pharmacy residency at the University
of Pittsburgh Medical Center McKeesport Hospital. Dr. Sabella completed
a Mastery of Teaching Certificate through the University of Pittsburgh

Eva Coulson, PharmD, BCPS
Eva Coulson, PharmD, BCPS is a Pain Management Clinical Pharmacist at Kaweah
Health. She serves as a preceptor for PGY1 pharmacy residents and pharmacy
students from the University of California, San Francisco. Dr. Coulson
joined the pharmacy team as a pain management specialist after completing
her PGY2 Pain Management and Palliative Care residency at the University
of Iowa College of Pharmacy in Iowa City, Iowa. She completed her PGY1

Kellie Bartlett, PharmD
Dr. Bartlett is a Clinical Pharmacist at Kaweah Health and serves as an
internal medicine and emergency medicine preceptor for both PGY1 pharmacy

at the University of California, San Francisco School of Pharmacy (UCSF).
Dr. Bartlett completed her Bachelor of Science degree in Biochemistry
from the University of St. Thomas (St. Paul, MN), then she attended the
University Of Minnesota College Of Pharmacy in Minneapolis, MN and received

Karly Erickson, PharmD, BCPS, BCCCP
Karly Erickson, PharmD, BCPS, BCCCP is an Emergency Medicine Clinical Pharmacist
at Kaweah Health. Dr. Erickson precepts both PGY1 pharmacy residents and

Kelsey Zapata, PharmD, BCPS, BCCCP
Kelsey Zapata, PharmD, BCPS is an Emergency Medicine Clinical Pharmacist

Dr. Zapata received her doctor of pharmacy degree at the University of

Jessica Le-Nieves, PharmD
Jessica Le-Nieves, PharmD is a Clinical Pharmacist at Kaweah Health Medical

Infectious Disease
Justin K. Ma, PharmD, BCIDP
Justin K. Ma, PharmD, is an infectious disease (ID) specialist who serves
as the antimicrobial stewardship pharmacist for Kaweah Health. He also
serves as the preceptor for the ID rotation for PGY-1 pharmacy residents
and pharmacy students from California and Arizona. Dr. Ma received his
Bachelor of Science in Pharmaceutical Sciences from the University of
California, Irvine in 2017 and his Doctor of Pharmacy from Midwestern
University College of Pharmacy - Glendale in Arizona (AZ) in 2020, where
he also received his teaching certificate. He then completed a PGY-1 Acute
Care pharmacy residency at Dignity Health - Yavapai Regional Medical Center
in Prescott, AZ in 2021 and a PGY-2 Infectious Diseases pharmacy residency
at Yale New Haven Hospital in New Haven, Connecticut in 2022. His professional

Anemia Management
Coleen Moriarity-Suggs, PharmD, BCPS
Coleen Moriarity-Suggs, PharmD is a clinical pharmacist specializing in
dialysis and anemia management at Kaweah Health. She received her Doctor
of Pharmacy degree from the University of the Pacific School of Pharmacy.
Dr. Moriarity-Suggs serves as an Assistant Clinical Professor for University
of California San Francisco School of Pharmacy and precepts PGY-1 pharmacy
residents as well as students from various schools of pharmacy. She was
the recipient of the UCSF Apple for Excellence in Teaching Award for the
UCSF Fresno site in 2014 and 2021. The KDMC Pharmacy Anemia Management
program under the direction of Dr. Moriarity-Suggs was awarded the ASHP

Kyra Ashcraft, PharmD, BCPS, Pediatrics / NICU
Kyra Ashcraft, PharmD., BCPS is an adult/pediatric/NICU clinical pharmacist

and pharmacy students from the University of California, san Francisco
and other pharmacy schools. Dr. Ashcraft completed her PGY-1 residency

Tauhid Bhuiyan, PharmD, BCPS
Tauhid Bhuiyan, PharmD, BCPS, is a Clinical Pharmacist at Kaweah Health.
He serves as a preceptor for PGY1 residents as well as students from the
University of California, San Francisco and other pharmacy schools. Dr.
Bhuiyan, received his Bachelor of Science degree in Biology from Wayne
State University in Michigan followed by his Doctor of Pharmacy degree
from University of New England College of Pharmacy in Portland, Maine.
He completed a two-year PGY1 residency from King Faisal Specialist Hospital
and Research Center in Riyadh, Saudi Arabia followed by serving as a NICU
Clinical Pharmacist at the same hospital. He served as an Assistant Professor
for three years at American University of Health Sciences College of Pharmacy
