Andrea Callas, PharmD
Andrea Callas is an Ambulatory Care Clinical Pharmacist at Kaweah Health.
She currently practices in the outpatient setting at the Kaweah Health
Specialty Clinic and sees patients enrolled in the Employee Wellness Program.
This multidisciplinary program focuses on chronic disease management for
employees and their dependents with eligible chronic conditions, such
as diabetes type 1 and 2, rheumatoid arthritis, chronic infectious diseases,
and chronic obstructive pulmonary disease (COPD). She received her Doctor
of Pharmacy degree from Midwestern University College of Pharmacy-Glendale.
Dr. Callas completed a PGY1 pharmacy residency with a focus in ambulatory
care at Marshfield Medical Center in Marshfield, Wisconsin, followed by
a PGY2 residency in geriatrics and academia at Midwestern University College
of Pharmacy-Glendale. Her areas of interest include ambulatory care, geriatrics,
and academia.

Melanie Vela, PharmD
Melanie is an Ambulatory Care Clinical Pharmacist at Kaweah Health. She received her Doctor of Pharmacy degree from Marshall B Ketchum University in 2022 followed by a community-based PGY1 residency with Safeway/University of California San Francisco and a PGY2 in Ambulatory Care Pharmacy with Kaweah Health. She has a passion for serving the underserved communities and helping to bridge the gap in health disparities, so she decided to stay with Kaweah Health. She currently practices as an ambulatory care clinical pharmacist at one of Kaweah Health’s rural health clinics, Kaweah Health Exeter Clinic. Cory Nelson, PharmD, BCACP
Cory is the Ambulatory Pharmacy Manager and the Residency Program Director for the PGY2 Ambulatory Care Pharmacy Residency. In this role he manages the ambulatory care service lines across Kaweah Health’s outpatient clinics. He also assists the clinics with their medication use processes and assists Clint Brown with his outpatient pharmacy duties. Cory practices comprehensive medication management at the various outpatient clinics as needed. Prior to this role, Cory practiced as an ambulatory care pharmacist with Kaweah Health. He received his Doctor of Pharmacy from Idaho State University College of Pharmacy in 2014 and completed the two-year Pharmaceutical Care Leadership Residency with the University of Minnesota in 2016. Clint Brown, PharmD
Clint Brown, PharmD, is the Assistant Director of Pharmacy overseeing the outpatient pharmacy services for Kaweah Health. The outpatient services include and retail pharmacy with a meds to beds program, an employee pharmacy, a home infusion pharmacy, and an ambulatory care pharmacy team. Prior to this he was an Oncology and Pain Management Clinical Pharmacist at Kaweah Health for eight years. Dr. Brown completed his PGY1 pharmacy residency in 2009 with the Veterans Affairs Ambulatory Clinic in Pocatello, Idaho. He received his doctor of pharmacy degree from Idaho State University in 2008. In 2014 Dr. Brown and the Pharmacy Pain Management Service were recipients of the Best Practice Award from the American Society of Health-System Pharmacists (ASHP) for their work with implementing a pharmacy driven pain management service in the inpatient setting and also received recognition from the California Society of Health-System Pharmacists (CSHP) highlighting this service as an Innovative Pharmacy Practice. Pranav Amin, PharmD
Pranav Amin, PharmD, is an Ambulatory Care Clinical Pharmacist at Kaweah Health Specialty Clinic Center. He serves as a preceptor for the PGY-1/PGY-2 Ambulatory Care pharmacy residency programs and APPE pharmacy rotation students. He received his Doctor of Pharmacy degree from the University of Arkansas for Medical Sciences College of Pharmacy in Little Rock in May 2020 and completed his PGY-1 Community-Based (2021) and PGY-2 Ambulatory Care (2022) pharmacy residencies at ARcare, a Federally Qualified Health Center, serving the rural populations around central and north-central Arkansas. Bradlee Rea, PharmD
Dr. Rea is a Clinical Pain Pharmacist at Kaweah Health. He practices in the outpatient setting at the Kaweah Health Specialty Clinic Center and helps by providing recommendations to practitioners who have patients dealing with chronic pain in some of the districts rural health clinics. Dr. Rea functions as the primary preceptor for the PGY-2 ambulatory care residency pain rotation (Pain I) and as a secondary preceptor to the advanced pain rotation (Pain II). He received his Doctor of Pharmacy from Southern Illinois University Edwardsville School of Pharmacy in 2015 followed by a PGY-1 residency at HSHS St. Elizabeth’s Hospital in 2016 and a PGY-2 Pain and Palliative Care Residency at Hospice of Southern Illinois in 2017. Here he practiced primarily in a community hospice organization with additional experiences in the treatment of outpatient chronic pain. Coleen Moriarity-Suggs, PharmD, BCPSColeen Moriarity-Suggs, PharmD is an Anemia Management Clinical Pharmacist at Kaweah Health. She received her doctor of pharmacy degree from the University of the Pacific School of Pharmacy. Dr. Moriarity-Suggs serves as an Assistant Clinical Professor for University of California San Francisco School of Pharmacy and precepts PGY-1 pharmacy residents as well as students from various schools of pharmacy. She was the recipient of the UCSF Apple for Excellence in Teaching Award for the UCSF Fresno site in 2013. Samantha Valle-Oseguera, PharmD, BCACP, BCGP
Dr. Valle-Oseguera is an Associate Clinical Professor and Experiential Program Director in the Department of Clinical Pharmacy at UCSF School of Pharmacy. Her clinical practice site is at Kaweah Health Medical Center in Visalia, California, where she serves as an ambulatory care clinical pharmacist at the primary care rural clinics. She serves as a preceptor for Kaweah Health's PGY2 Ambulatory Care residents, PGY1 residents on an ambulatory care rotation, and APPE pharmacy students from UCSF rotating through the Fresno area. She graduated from the University of the Pacific in 2012 with her Doctor of Pharmacy degree. She completed a PGY1 and a PGY2 in Ambulatory Care at the Boise VA Medical Center. Eva G. Escobedo, PharmD
Dr. Escobedo is an Ambulatory Care Clinical Pharmacist at Kaweah Health. She practices outpatient at Sequoia Health and Wellness Centers and inpatient at Kaweah Health Medical Center. She serves as a preceptor for APPE students on Ambulatory Care rotation as well as PGY1 and PGY2 residents on Family Medicine rotation. Additionally, she serves as staff for the Kaweah Health Family Medicine Residency Program. She received her Doctor of Pharmacy degree from Manchester University College of Pharmacy, Natural & Health Sciences in Indiana in 2020 followed by a PGY1 in Ambulatory Care Pharmacy with the University of Southern California School of Pharmacy.
